WELCOME TO OUR PRACTICE

Here at Third Avenue Doctors Surgery we have two G.P. Partners, one regular female GP, a GP Pharmacist, two Practice Nurses and a Healthcare Assistant. We also have the use of Health Professionals employed by the Primary Care Network, being GP Pharmacists, Mental Health Workers, Wellbeing Link Co-ordinators and Emergency Care Practitioners. We are not a training practice. Our Practice area covers the whole of Canvey Island and disabled access/toilets are available.
We employ regular locum G.P.s to see patients to cover annual leave, etc.
Our Practice Manager runs our Practice with the help of nine reception staff.
If you would like to join our Practice please make an appointment for a pre-registration check.
Our opening hours are 8.00 – 6.30p.m. Monday to Friday, with various bookable appointments throughout the day with all clinicians. All appointments are bookable from 8.30a.m. Monday to Friday. We will see patients in an emergency, but please telephone in advance. If patients just turn up, you may be sent away to return at a later time during that day. We offer telephone appointments as well as face to face appointments.
Patients are able to choose which Doctor they would like to see for a bookable appointment, but you may not have a choice in an emergency. Our Care Navigators will ask you why you need to see a health professional, in order for us to book you in with the most appropriate clinician.
Appointments are precious and we would encourage you to be punctual for your appointment and cancel any appointments that are not wanted.
Emergencies outside normal working hours are covered by an out of hours service. If you have an urgent need to see a Doctor when we are closed, please telephone 111 or telephone the Practice in the normal way and a recorded message will tell you the number for the out of hours service.
Weekend and evening pre-bookable appointments are available. These may not be with your registered GP, but could be with another Clinician. At the time of booking one of these appointments, the Receptionists will advise you of the Clinician you will see and the location to be seen at. These could also be telephone appointments. Locations may vary, but will be within the Castle Point and Rochford area.
Appointments are also provided by our PCN, who run clinics at their offices within the Primary Care Centre, Long Road, Canvey Island. These are emergency appointment on the day and will be offered to you by our Care Navigators.
Home visits will only be carried out at the discretion of the Doctor. Transport problems are not an acceptable reason for requesting a home visit, home visits are for patients who are housebound. If you cannot attend the Surgery, please telephone before 11.00a.m. Monday to Friday, the Receptionist will take your details and the Doctor on duty may telephone you back later during that morning. Home visits are not only carried out by the Duty Doctor, but also by the Emergency Care Practitioners.
Repeat Prescriptions can be requested in writing. Please allow 48 working day hours for collection. We also offer an online service for requesting repeat prescriptions. Please visit our website at thirdavenuehealthcentre.nhs.uk. If you wish your prescription can be sent electronically to a nominated pharmacy of your choice. Ask a Receptionist for full details. Alternatively, you can order prescriptions on the NHS App.
Online Access to Medical Records is available to patients. If you would like to order a repeat prescription or access your summary care record, please attend the Surgery with photo ID and a utility bill showing your current address. A member of our Team will then be able to register you for this service.
We offer a full range of contraception services, including the fitting of coils and Implant fitting. We offer cervical screening, maternity services, joint injections and Childhood vaccinations. You can also make an appointment to see our Smoking Cessation Nurse for stop smoking advice.
Travel Vaccinations can be done at our Surgery. Please complete a travel assessment questionnaire before making an appointment with a Practice Nurse who will then be able to advise on up to date vaccine cover for the country you are visiting.
Chronic Disease Management Clinics i.e. diabetes, coronary heart disease, asthma, chronic kidney disease etc are carried out within our surgery hours.
Chaperone Policy – Our policy is displayed within our Surgery.
Confidentiality - Confidentiality of patients’ records is maintained at the highest level by all staff. All patient records on file or computer are confidential and in order to maintain confidentiality, patient information will only be given to patients themselves, parents of minors, if appropriate or with written consent, from the patient to a named individual.
Complaints/Compliments - If you have any comments or suggestions regarding our Practice put them in writing to Mrs Faye Graham, our Practice Manager. We follow the NHS Code of Conduct and deal with complaints according to this in the following manner:
- Complaints are acknowledged within 3 working days which will include an offer to discuss the matter with an appropriate person in the Practice.
- A complaint will have to made within 12 months of the incident occurring or within 12 months of the date on which the matter came to the notice of the complainant.
- Any complaints that remain unresolved after 6 months will be reviewed to ensure that all is being done to attempt to resolve the case.
